As a new patient, here is what you can expect on your first visit and future visits if they are needed.

Initial office visit

•  60-minute appointment
Your initial appointment includes a confidential interview to discuss your symptoms and medical history. A physical exam will follow to determine your treatment plan, and treatment will begin.

Follow-up visits

•  30-minute appointment
On your follow-up visit, we’ll review your history and progress followed by another hands-on treatment.

Sports physicals are available for high school sports.

PAYMENT OPTIONS

Cash, Check, MasterCard or Visa

Payment is expected at the time of visit.

Insurance

If you would like to bill an insurance company, I will give you a receipt to turn into your insurance company.

I do not have the ability to bill insurance companies. Your agreement with your insurance is between you and your insurance company, they are much more responsive when billing or any communication comes from you directly. Hence, I will give you a receipt which includes the proper procedural and treatment codes to send to your insurance company. They will pay you back according to your agreement with them.

If yours is a personal injury case involving a car accident etc., please let me know at the time we book your first appointment. There are other forms you need to fill out to cover you if needed at a future time.

I’m sorry I do not have the resources to bill Workers Comp. or Medicare. If either of these applies to you, please let me know when you call the office.

APPOINTMENTS & CANCELLATIONS

A specific day and time is set aside for your appointment. In the event that you cannot keep your appointment, please notify me 24 hours beforehand. Failure to notify me will result in a charge for the missed appointment.

Tuesday and Thursday: 9am — 5:30pm
Wednesday: 2pm — 7pm
Saturday: 9am — 4pm
